Financial Policy

Any co-pays or individual deductibles MUST be paid at the time of treatment. We accept cash, check, VISA, MasterCard, Discover, and Care Credit. If your treatment involves more than one visit, your fee is expected at the first appointment. If you have dental insurance, we will submit your claims as a courtesy to you. Our office collects your estimate co-pay provided to us by your dental insurance, at the time of treatment. Dental insurance is a benefit that your employer provides for you, or you are providing for yourself. Our staff will do their best to determine your coverage, with the understanding that insurance plans are always changing, and it is your responsibility to know your plan. A quote of eligibility from your insurance company does not guarantee coverage at the time of service. ***ALL BALANCES THAT HAVE NOT BEEN PAID WITHIN 90 DAYS FROM YOUR INSURANCE WILL BE TRANSFERRED TO YOUR ACCOUNT.

Returned checks are subject to a $25 NSF fee.

Any account delinquent more than 90 days will be subject to a monthly late fee.

No Show & Canceled Appointment Policy

A $50 fee will be added to your account for any no show and/or canceled appointments without 24-hour notice. Repeated no show/canceled appointments will result in dismissal from our practice.
